This goal setting worksheet will help you plan the actions you need to take to reach your goals and acts as a visual tool to help you stay on track. 

Trigger Tracker

During my journey with Bipolar and Anxiety Disorder, on of my favourite and most useful tools was a trigger tracker. 


This worksheet prompts you to record physical feelings and thoughts connected to an event to help you better understand the common triggers of things like anxiety or depression so you can better manage them in the future.

Monthly Mood Chart
Another of my favourite tools has been my monthly mood chart. Used in conjunction with the trigger tracker, it is a great way to see patterns and to properly manage them.
